“At Pathway Recuperative Care, our nursing staff provides
medical oversight which means we keep a close watch on their health to ensure
they are healing appropriately and that all of their follow-up medical
appointments are set and met,” shared Kelly Bruno, President and CEO of
National Health Foundation (NHF), “more importantly, we care. We care deeply
that our clients have an interruption in homelessness and if this triggers a
desire to be connected with services to find their way into a permanent and
stable housing situation, we walk that journey with them as well. But it is our
exceptional nursing staff that is on the front line of delivering that
life-changing care.”</div>

For Denise and her counterpart at Pathway Recuperative
Care’s San Gabriel Valley location, Tammy Grimes, LVN, a typical day involves caring for
numerous clients while working with social workers and support staff as stable
housing is secured.</div>

“I get so excited when we have arranged for housing for a
client. It gives them the opportunity to focus on something other than where
they will lay their head each night,” shared Tammy. For Denise, the joy of
seeing the smile on an individual’s face when they receive housing is the
favorite part of her job.</div>

Working with individuals who are without a home has brought
Denise and Tammy face to face with the most common misconceptions about this
population, “I see people fearing those without a home and that is something I
would love to change,” shared Denise. For Tammy, the notion that everyone who
is without a home is on drugs or alcohol is far from the truth, “ The truth is
everyone has a story and they are all different.”</div>

Denise is inspired by her clients’ tenacity, “I’m always
humbled by the people I work with. They are willing to try and start over from
scratch.” Tammy’s thoughts turn to the fundamental need for housing, “Living
conditions make treating and coping with health issues more difficult. Health
cannot be a priority if the individual is focused on their safety and where
they will sleep at night. Mortality is higher living on the street.” Both
nurses have one wish for their clients: Stable housing in a community that is
safe and loving. </div>

For Bruno, having exceptional staff working with clients has
raised the bar of what compassionate care looks like, “On behalf of all of us

Cedric has been homeless for 6 months and has spent
nearly half of that time in the Hospital. A liver transplant and a subsequent
bout with pneumonia left the young man weakened and in need of care that
extended well beyond his hospital stay. Through a partnership with National
Health Foundation (NHF) and a local hospital, Cedric was able to receive
continued care at NHF’s Pathway Recuperative Care. </div>

Like many individuals who have undergone major surgery, Cedric had countless follow-up appointments he needed to make and manage. For
individuals who have a home and resources such as a car and family support,
such care is challenging, but manageable. For individuals without a home, the
demands of post-operative care are rarely met and subsequent hospitalizations
are frequently necessary.</div>

<div class="MsoNormal">
With the help of staff at Pathway Recuperative Care, Cedric attended all of his appointments and his health has taken a turn for the
better. His strength returning, the staff then turned their attention to
connecting Cedric with all of the resources available to him and to finding a
permanent and stable housing situation.<span style="mso-spacerun: yes;">
</span>“The concept behind Pathway Recuperative Care is to provide care to the
whole person,” shared Shakoya Green, Pathway Recuperative Care Program Director.
“First we provide a safe and comfortable temporary home with medical oversight
so that the patient may heal physically. Then we look at the situation that led
to the individual losing their home and their health. By addressing the social
determinants of health that challenged the individual, we can arrive at a path
to health.” For Cedric, there was a need to connect him with the Department of
Public Social Services to apply for financial benefits. Then staff worked with Cedric to secure a permanent home. He was connected with his birth mother in
New Jersey who flew to Los Angeles to bring her son home.</div>

When asked how he felt about his nearly 2-month stay at
Pathway Recuperative Care Cedric responded, “ I am feeling incredibly blessed to
have been at Pathway. After being in the hospital as long as I have, it was
nice to reconnect with people. After getting a liver transplant I feel like I
got a second chance and Pathway helped me on my way.” Green added, “Pathway
Recuperative Care demonstrates and puts into action the idea that home is
health. Without a ‘home’, a person cannot thrive and be healthy. We are so
pleased that we were able to connect Cedric with his family and walk with him
on this part of his journey.”</div>

research has identified a significant link between education and health. High
school graduates live longer, healthier lives than dropouts. College graduates
have even longer life spans, better access to health care, better dietary and
health practices, and overall better health. Unfortunately, about a quarter of
California counties have high school dropout rates close to 20 percent or more,
including Los Angeles at 17 percent. While there are numerous factors
contributing to the dropout rate, one third of female dropouts say that
pregnancy or becoming a parent played a role in their decision to leave school.
Girls most at risk for teen pregnancy are Latinas from low income communities.
There is critical need for education and support services to reach these adolescent
girls at the greatest risk for pregnancy and subsequent dropout before they get
pregnant, ultimately breaking the cycle of teenage pregnancy. </span></div>

Health Foundation has implemented the Go Harold’s Way; Be a STAR (Successful
Teens Acting Responsibly) Girls Program. Goals of this program include reducing
the incidence of pregnancy, improving high school graduation/completion and
connecting participants to health and social services and resources in their
community. Since its implementation, NHF has provided more than 700 students
with the tools and education they need to prevent unwanted pregnancies and
abilities to make healthy life choices. </span></div>

<span style="background: white; color: #666666; font-size: 12.0pt; line-height: 200%;">Haydee and Yvonne are two participants of the Be a STAR Girls
program at Ramona High School and are set to graduate in 2018. For Haydee, the
program gave her key information she did not have access to beforehand, “I
learned there’s many ways to prevent pregnancy and learned all the parts of the
female anatomy and what they do. I feel confident about my reproductive health
because I’m more informed about different topics that we learned in class.” The
weekly topics touched on birth control and STDs as well as how to plan for the
future. Yvonne admitted that before being in the program, ‘ditching school’ was
a chronic issue. “Before coming to Ramona I was ditching a lot of school and
now I come to class every day and will graduate next year.<i style="mso-bidi-font-style: normal;"> </i>My plans after graduating school are to get a job and go to
community college to help me have a career later in life.”<i style="mso-bidi-font-style: normal;"> </i>In addition to committing to her future, Yvonne has a deeper
appreciation for her body and her health, “In the Be a STAR class we got to
talk about our physical bodies and how a woman’s body looks on the inside. We
also lean to be more confident about our bodies. I now know how to protect
myself from getting an STD.”</span><span style="background: white; color: #666666; font-size: 12.0pt; line-height: 200%;">When not in school Yvonne enjoys playing basketball with
friends while Haydee likes to try new things. Both students have expressed that
this program has given them the confidence to be able to make healthy decisions
when it comes to their reproductive health. After graduation, both Haydee and
Yvonne plan to attend community college laying the path for their future
careers.</span>

an interest in conquering his substance abuse issues. As an individual who was
without a home, Sharonda knew her patient could qualify for recuperative care,
but would that be enough to keep him healthy and safe until room became
available at a substance abuse program?</span></div>

Sharonda is one of the point people who, at the end of a patient’s stay in the
hospital, sees to it that they are safely discharged. When a patient is
discharged and they do not have a home to return to, Sharonda finds alternative
solutions based on the patient’s most pressing needs. Some times the needs and
the available services do not synch. Robert was a candidate for recuperative
care and for the substance abuse program, but there was a significant gap
between when his stay in recuperative care would end and the substance abuse
program could take him in. The risk Robert was facing was a return to the
streets and possibly missing the opportunity for the rehabilitation that he
wanted to receive. </span></div>

Recuperative Care hospital partner, White Memorial Medical Center works hand in
hand with Pathway staff to ensure the best possible outcomes for each client.
For Robert, Pathway staff advocated on his behalf and they were able to secure
additional time for him to stay at Pathway until a space became available for
him at the substance abuse program. </span></div>

and options has given Sharonda a unique perspective on working with this

March is National Nutrition Month. It is also the month that
National Health Foundation is beginning to offer nutrition education classes in
Historic South Los Angeles. As part of the Champions for Change <i style="mso-bidi-font-style: normal;">Healthy Communities Initiative</i> grant
received from the Los Angeles County Department of Public Health, NHF will be
conducting 30 minute to 1 hour long nutrition education classes at schools in
the community with students and parents. Led by our trained and expert staff,
these classes are intended to educate South LA residents about the importance
of healthy eating and physical activity. Classes will cover topics such as the
USDA’s MyPlate guidelines, the importance of drinking water, reading and
understanding food labels, shopping for healthy food on a budget and everyday
activities that can help maintain weight and fend off disease. </div>

<div class="MsoNormal">
These classes are one part of the healthy story that is
emerging from within South LA.<span style="mso-spacerun: yes;"> </span>Our
Health Academy students have worked with two, and are currently assessing 8,
local food markets to see how they advertise and sell healthy options. They
collaborate with store owners to improve the visibility of healthy foods, such
as fruits and vegetables, and offer help with marketing these options to the
community. Their efforts are working! More healthy foods are visible to
consumers and one market owner has even changed his food procurement process to
receive healthy food from a local produce distributor, making these healthy
options more available and cheaper to the community.<span style="mso-spacerun: yes;"> </span></div>

Together, these two approaches are intended to tackle the
social determinant of health that is access to healthy and nutritious food.
South LA is considered a food desert with few healthy food options available.
Furthermore, food insecurity is an issue that many in the community face. The
nutrition education classes are being offered with the expectation of shifting
the trend of obesity and heart disease in the community. By reinforcing the positive changes happening in South LA, including
the availability of fresh produce at corner markets, we will ensure that every
individual and every family has the access to the knowledge and ability to eat healthy and
live their best, healthiest life. </div>

fewer visits to the emergency department for her patients. Nancy works with
patients who are about to be discharged from the hospital and the majority of
her caseload is individuals who are without a home. </span></div>

<span style="font-family: "calibri"; mso-ascii-theme-font: major-latin; mso-hansi-theme-font: major-latin;">“I see myself as someone who advocates for my
patients and collaborates with the nurses, doctors and case managers to work
towards a safe discharge,” shared Nancy, “The challenge is that when someone is
not able to heal in a clean and safe environment, it puts the person at a much
greater risk of another hospitalization and further exacerbating their illness,
all of which can be prevented.”</span></div>

<span style="font-family: "calibri"; mso-ascii-theme-font: major-latin; mso-hansi-theme-font: major-latin;">Since 2015, National Health Foundation’s
Pathways Recuperative Care has become one avenue for safe discharge of homeless
patients from partner hospitals. Pathways offers temporary housing with medical
oversight to individuals who are without a home. The advantage for patients is
that they can heal safely and are given the support needed to adhere to follow-up
care schedules. For partnering hospitals the advantage is peace of mind, “The
hospital feels reassured that the patients are able to go somewhere clean and
safe to heal while getting assistance with follow-up appointments and housing,”
says Nancy.</span></div>

<span style="font-family: "calibri"; mso-ascii-theme-font: major-latin; mso-hansi-theme-font: major-latin;">Not every homeless patient opts for accepting
Recuperative Care and not all are a fit. As a social worker, Nancy assesses
each patient who is homeless to see if they are appropriate for recuperative
care, if they are motivated to get housing, and those who wish to recover from
their medical condition. Not all homeless patients want to be housed, “Personally,
it makes me sad because it doesn’t have to be that way. But I also know that we
all have rights to make choices that we feel is best for ourselves and to choose
to take help when it is offered.”</span></div>

prioritizing between a victim of crime, someone who is homeless and being
discharged while it is cold and raining outside, and another who is dealing
with a life changing/altering diagnosis. We at National Health Foundation’s

who are providing essential care to patients at their most vulnerable; upon

Ron Jacobs, LCSW, Social Work Services at <a href="https://www.adventisthealth.org/white-memorial/pages/default.aspx" target="_blank">White MemorialMedical Center (WMMC)</a> in Los Angeles, works closely with patients as they
prepare for discharge from the hospital. Nearly every day Ron is faced with the
challenge of discharging a patient who does not have a home to return to.<span style="mso-spacerun: yes;"> </span></div>

“I make sure that those I serve are listened to and heard. I
motivate individuals to enhance their strengths and abilities and help restore
their sense of dignity while avoiding stigmatizing anyone as homeless,” says
Ron of his role, “and I also realize that along with empathy, there must be
action, resources and referrals.</div>

March is #SocialWorkMonth and <a href="http://nhfca.org/" target="_blank">National Health Foundation(NHF)</a> is celebrating the invaluable work that social workers do in the
community, in particular the work of those who coordinate the discharge and
referral of patients who are homeless.</div>

<span style="mso-spacerun: yes;"> </span>Until 2015, patients
in Los Angeles County who were without a home and were discharged from the
hospital faced completing their healing on the street. For many, a lack of
follow-up care meant a certain return to the hospital. For social workers like
Ron, it meant forming a connection with an individual upon discharge only to
see them back in the hospital. “Knowing that there is a resource that is an
alternative to the streets is a wonderful advantage we can now offer these
individuals,” shared Ron, “Pathway Recuperative Care provides the opportunity
for transitional healing, emotional support, resources and concrete solutions
to enable our patients to reconnect with their community.”</div>

Shakoya Green, MA, MSW, National Health Foundation’s Recuperative
Care Program Director had this to say about Ron, “We simply cannot do what we
do without Social Workers like Ron who go above and beyond their ‘job’ to
really connect with their patients on a deeper level. Ron sees the whole person
and addresses their needs from a place of love. We have been able to work
closely together to achieve some incredible outcomes for our patients.”</div>

Frank was discharged from WMMC last fall after several
complex procedures. Ron ensured a smooth referral to Pathway Recuperative Care
and helped to secure every possible form of care to ensure Frank recovered from
his physical issues. Frank was able to benefit from the full spectrum of
services that the partnership provided and has since fully healed physically
and has been provided with a stable housing solution. “Frank’s case exemplifies
what is possible when there is a ‘champion’ for the homeless who sees beyond
the sometimes harsh exterior and works compassionately and diligently to ensure
success,” added Shakoya.</div>

Ron’s motto is a moving one, “I strive to meet each person
as he/she is and I begin where the person is at in their journey in life. I try
to remember that we are all ‘poor’ in one way or another and we all need some
degree of healing at some time in our lives. There is no ‘healer and would be
healed’. When we help someone in need, we are all better off. We all share a
common humanity.” </div>

John A. Calderone, Ph.D., former Chief Executive Officer for

Association of Southern California for over fifteen years, its Executive
Committee for seven years, and as Board Chairman in 2014. He served on the
Board of Trustees and its Executive Committee for the California Hospital
Association. His personal experience of modest beginnings fostered his deep
belief in educational mentorship, giving back to the community, assisting those
less fortunate, and a sincere commitment to helping individuals who are
experiencing homelessness.</div>

Paul Lopez is the Senior Vice President, National Strategy

populations. His primary goal in life and work is to find innovative solutions
that bridge gaps in access and quality care. In the healthcare arena, Paul
successfully leads teams that deliver quality healthcare and programs to those
most in need. His personal values, motivations, and compassion for those in
need are closely aligned with National Health Foundation’s mission and vision.</div>

Jeffrey L. Thompson currently serves as Associate Partner within IBM's

Flor’s Mini Market is a neighborhood corner store that is
one block from Thomas Jefferson High School in Historic South Central Los
Angeles. It’s a favorite among high school students who frequently stop by to
purchase a bite to eat before class or during lunchtime. Her merchandise mix is
typical of a ‘tienda’ with must have home and personal products as well as some
food and beverage inventory. Until recently, the food for sale was mostly packaged
with few healthy food options. In a community that is both a food desert (with
few healthy food retail choices) and a food swamp (categorized by a prevalence
of unhealthy food outlets), markets such as Flor’s have come to represent
unique opportunities and key potential partners in the effort to improve access
to healthy, affordable foods in South Los Angeles.</div>

Recently, Flor joined the <a href="http://comprafoods.org/">Community
Markets Purchasing Real and Affordable Foods (COMPRA foods)</a> program, an
innovative food distribution system that caters to small businesses that want
to offer healthy food and leverages their collective purchases to secure better
prices and quality products from wholesalers. During the first delivery of fresh
produce last week, Flor was approached by a dynamic group of high school students
known as the <a href="http://southlachl.weebly.com/">Community Health Liaisons
(CHL)</a>. As part of<span style="mso-spacerun: yes;"> </span><a href="https://www.blogger.com/nhfca.org">National
Health Foundation’s (NHF’s)</a> <a href="http://buildhealthchallenge.org/">BUILD
Health L.A. Initiative</a>, CHL youth<span style="mso-spacerun: yes;"> </span><span style="mso-spacerun: yes;"> </span></div>

work with neighborhood markets to help corner store owners
introduce healthy produce, and better merchandise fresh fruits and vegetables.
CHL’s help store owners make small, gradual changes by providing technical
assistance that can help overcome the challenge of introducing new inventory
and taking on new risk. </div>

Two CHL youth, Ariana Vega and Melissa Villarruel, ‘adopted’
Flor’s Mini Market to help the store owner adjust to the demands of selling
perishable healthy produce and help to ensure changes are profitable and
sustainable, with the overall goal of increasing the availability of fresh,
quality fruits and vegetables in their neighborhood. During this initial
meeting, the CHL’s helped Flor inventory produce shipment and begin using
marketing strategies to promote fruits and vegetables in-store. <span style="mso-spacerun: yes;"> </span>Together they carved out space in the store
for the produce and used baskets and other materials to entice consumers to
consider adding fresh fruits and vegetables to their purchases. Within a week,
the students will check back with Flor see how things are going and modify
their strategies as needed.</div>

The students are keenly aware of the alarming prevalence of obesity,
heart disease and diabetes in their community. For NHF, the BUILD Health Initiative
presents an opportunity for partners to come together and improve the health of
the community from within. By working with local high schools, NHF is able to
directly connect with the energy of youth from the community. NHF brings CHL
youth together with community partners such as <a href="http://lurnetwork.org/">Leadership
for Urban Renewal Network (LURN),</a> Los Angeles Food Policy Council, and Los
Angeles Department of Public Health among others to share knowledge and to
craft innovative solutions to build a healthier South Los Angeles overall. <span style="mso-spacerun: yes;"> </span>The students will continue to monitor their efforts
to glean important outcomes, however, their efforts have already been well
received by storeowners and the community alike.</div>

For Ariana and Melissa, the experience of working with Flor
left them inspired, “<span style="color: #454545; mso-bidi-font-family: "Times New Roman"; mso-fareast-font-family: "Times New Roman";">I felt really good
knowing that we would be able to help her not only to sell her produce but also
some of the other merchandise in her store,” shared Ariana. Melissa had this to
say about her experience, “The level of excitement I had from helping Flor was
inexplicable, it felt as if I were riding a roller coaster, or the feeling when
you just learn to drive; it felt great!”</span></div>

Jazz Park in Historic South Central Los Angeles was
transformed into an outdoor fitness space with community members and
instructors moving through workouts designed to increase cardiovascular
capacity, boost energy levels and increase the feeling of wellbeing. For
community member Claudia Durango, it was a welcomed event, “This is going to be
really good for the community; we just need to continue having people show up.”
The new program is part of the rollout of <a href="http://nhfca.org/">National Health
Foundation’s (NHF</a>) <a href="http://buildhealthchallenge.org/">BUILD Health
LA Initiative</a> Community Action Plan for Historic South Central.</div>
<div class="MsoNormal">
In 2016, NHF’s BUILD Health LA Community Health Liaisons, a
group of local high school students and their mentors, surveyed the community
to understand residents’ barriers to health. The results of the survey were
captured in the Community Action Plan that aims to improve the health of the
community through methods that have been identified by the community’s
residents and that capitalize on partnerships and the assets available.</div>
<div class="MsoNormal">
Historic South Central Los Angeles is 6 square miles, and is
home to roughly 103,000 people. In this community, there are no gyms and there
is only 0.4 acres of park space for every 100,000 people. Along with having
limited access to open space, this community is considered both a food desert
(with few healthy food retail choices) and a food swamp (categorized by a
prevalence of unhealthy food outlets). Obesity and diabetes are common in this
community, and while physicians recommend healthy eating and exercise, these
lifestyle changes are challenging to practice with what is available in the community.

<div class="MsoNormal">
One of the needs the Community Action Plan addresses is for
programs to keep residents physically active. Many stated that the classes that
were available were too expensive and that the free programs had extensive
waitlists of residents eager to join. We learned that these residents want to
exercise and they will – if the opportunity is there. </div>
<div class="MsoNormal">
As a result, we partnered with <a href="http://www.3winsfitness.com/">3WINS Fitness</a>, an innovative program
spearheaded by Dr. Loy and Kinesiology students from California State
University, Northridge. 3WINS Fitness “adopts” parks in Los Angeles, and sends
Kinesiology students to teach community members how to exercise while nurturing
a community mentality that leads to friendships, accountability and results! </div>
<div class="MsoNormal">
In addition to 3WINS Fitness, NHF also partnered with <a href="http://www.dignityhealth.org/californiahospital/">California Hospital
Medical Center</a>, <a href="http://publichealth.lacounty.gov/">LA County Department
of Public Health</a>, <a href="http://www.laparks.org/">LA City Parks and
Recreation</a>, <a href="http://www.lapdonline.org/newton_community_police_station">Newton Community
Police Station</a>, <a href="https://allpeoplescc.org/">All People’s Community
Center</a>, and <a href="http://www.apch.org/">A Place Called Home</a>, to
launch the fitness program. Classes take place Mondays, Wednesdays, and Fridays
at Central Avenue Jazz Park and All People’s Community Center. </div>
<div class="MsoNormal">
Community members are thrilled with this new opportunity and
have already built great rapport with the instructors from 3WINS. For NHF, the
program represents a new way of delivering health to the community. “By
creating an opportunity for the community to express their needs and actively
engage in finding solutions to the health barriers they were experiencing, they
are bringing about healthy changes from within,” shared NHF CEO Kelly Bruno.</div>
<div class="MsoNormal">
Consistency is always the key with taking on a new behavior,
but we are sure that with this new partnership and with the different
components of the BUILD Health LA Initiative and Community Action Plan, the
residents in Historic South Central Los Angeles will have new opportunities to
live a healthier lifestyle. </div>

recruitment presentation in class when she was in 9<sup>th</sup> grade. It was
also Health Academy’s first year at Thomas Jefferson Senior High. “I wasn’t
really involved in anything else at that time,” she explains, “but Health
Academy just sounded different from all the other groups I’d heard about, so I
decided to go for it.” </span></span></div>

<span style="font-size: small;"><span style="font-family: Arial,Helvetica,sans-serif;">She went on to submit an application and sat through an
interview, and in the spring semester of 2014, Aracely became a member, or
“youth leader,” of the brand-new Health Academy program. After attending a few
meetings and experiencing first-hand what the program was about, she was
hooked. “At first I was debating whether to continue or not, but everyone in
the program was so nice and supportive. It felt like a good place to be.” </span></span></div>

<span style="font-size: small;"><span style="font-family: Arial,Helvetica,sans-serif;">Through Health Academy, Aracely learned some unexpected
things about her community. She learned how her neighborhood is often
considered a <i>food desert</i> or <i>food swamp</i> because of the prevalence
of fast food restaurants and liquor stores compared to full-service grocery
stores that sell fresh fruits and vegetables. While discussing food swamps, she
said, “it was definitely eye-opening. I thought that we already had a lot of
things in our community, but now I’ve realized that we don’t have as much as we
should.” </span></span></div>

<span style="font-size: small;"><span style="font-family: Arial,Helvetica,sans-serif;">Since realizing this, Aracely
has actively worked to improve access to healthy food for her school and her
community. She often shares information she learns through Health Academy with
her family and friends. “I always show my parents the healthy food recipes I
get from Health Academy, and we’ve started making them at home… I’ve also
taught my parents how to read nutrition labels, and it’s something we look at
now when we shop for food”. </span></span></div>

<span style="font-size: small;"><span style="font-family: Arial,Helvetica,sans-serif;"><span style="line-height: 107%;">Aracely
has also taken on leadership roles in several Health Academy initiatives, such
as implementing <a href="http://smarterlunchrooms.org/about-us">Smarter
Lunchroom</a> strategies in the school cafeteria and the <a href="http://nhfca-blog.blogspot.com/2016/09/usda-tours-national-health-foundations.html">Mini
Farm Stand</a> project. Throughout her participation, Aracely has been an
important voice for her fellow students and community members. One moment she
feels particularly proud of was attending a youth summit hosted by the Los
Angeles County Department of Public Health during her sophomore year, where she
presented her team’s Cafeteria Makeover project to other high school students
from across the County. “We made changes in the cafeteria that year that
decreased the wait times for getting lunch and also got students more excited
about the cafeteria food. More students started participating in the school
lunch program, and I like to think our project helped… I felt really proud of
how we presented our project and I think we were able to give other schools
some good ideas.”<br />

even more involved in school. Aside from Health Academy, she is now also an
active member of her school’s Gay-Straight Alliance chapter, Student Leadership
and ROTC. Now a senior, Aracely is in her fourth and final year in Health
Academy, where her role has expanded to include <i>Youth Social Media Manager</i>.
In this position, she will work with other Health Academy students to create
and post content for the <a href="https://www.instagram.com/nhfhealthacademy/">NHF
Health Academy Instagram page</a>. Aracely is also getting ready for college
next year and has already received acceptance letters from California State
University, Dominguez Hills and Loyola Marymount University. She aspires to
become a nurse someday, and credits her involvement in Health Academy for
inspiring her interest in the health field. </span></span><br />

<span style="font-size: small;"><span style="font-family: Arial,Helvetica,sans-serif;">Aracely is one of over a hundred students that has participated in Health Academy which is going into its fourth year through funding from the <a href="http://publichealth.lacounty.gov/" target="_blank">Los Angeles County Department of Public Health</a>. Recently, National Health Foundation received another Department of Public Health grant, the Champions for Change – Healthy Communities Initiative, to expand Health Academy to another Los Angeles high school; allowing NHF’s Health Academy and its proven benefits to reach many more students like Aracely, their families and the community. </span></span></div>

Arnie and Mat arrived at <a href="http://www.nhfca.org/">National
Health Foundation’s</a> (NHF) <a href="http://www.nhfca.org/pathawayrecupcare/">Pathway
Recuperative Care</a> from partner hospitals in early December 2016. Both were
in need of additional medical supervision and both were homeless. The
similarities in their cases ended there. The task of delivering essential care
and helping the two men find stable housing required an individualized plan
that took into account their stories, their needs and their goals.</div>

<div class="MsoNormal">
Arnie's multiple chronic medical conditions require monitoring and
medication, both of which are challenging to maintain on a modest monthly
social security check. He found himself unable to pay for rent and his
condition deteriorated until he needed to be hospitalized. Upon discharge from
the hospital he was homeless and in need of follow-up care. At <a href="http://www.nhfca.org/pathawayrecupcare/">Pathway Recuperative Care</a>,
Arnie worked closely with staff members on his care plan. This plan looks at
the patient’s needs both medically and with respect to housing. Staff then
began connecting Arnie to services available to him via the Department of
Mental Health as well as medical services to ensure continuous improvement on
both fronts. With Arnie’s motivation and Pathway staff persistence, he was able
to secure stable housing that will ensure a sustained return to health.</div>

For Mat, the struggle was no less intense. He came to <a href="http://www.nhfca.org/pathawayrecupcare/">Pathway Recuperative Care</a>
from a partner hospital with no money, no identification and no home. Struggling to recover
from substance abuse, the 20-year-old had spent years in the foster care system
and found emancipation to be more difficult than he had the skills to cope
with. Upon arriving at <a href="http://www.nhfca.org/pathawayrecupcare/">Pathway
Recuperative Care</a>, the staff worked with Mat to reapply for General Relief,
obtain a replacement Electronic Benefit Transfer card and kept him proactive in
attending the necessary medical and social services appointments that would
ensure his return to health. The last piece of Mat’s health plan was to secure
stable housing. Mat’s stable housing is such that he will receive the
support he needs to maintain his health while acquiring the skills he needs to
navigate adulthood. </div>

For <a href="http://www.nhfca.org/pathawayrecupcare/">Pathway
Recuperative Care</a> staff, each patient has a vastly different set of
circumstances that has led to the decline in their health and the loss of their
home. “Each person who comes to Pathway is seen as an individual with distinct
health needs. We take the time to understand their history and work with them
to devise a plan that matches their needs and capacity for managing their
progress to sustained health," explained Kelly Bruno, MSW, President/CEO <a href="http://nhfca.org/" target="_blank">National Health Foundation</a><span style="font-family: "arial"; mso-fareast-font-family: "Times New Roman";">.</span> For Arnie and Mat, the prospect of an end to
homelessness was welcomed with large grins and a sense of accomplishment. “We
find it impossible to speak about health without addressing the most
fundamental social determinant of health: housing. Without a home, there can be
no health,” asserts Bruno. With secure and supportive homes both Arnie and Mat
can now turn their attention and energy towards their healing and health.</div>

aims to reduce the prevalence of obesity among low-income Supplemental
Nutrition Assistance Program-Education eligible populations by providing
nutrition education, physical activity promotion, and working to create
healthier environments for low-income individuals and families where they live,
learn, work, play, pray, and shop. </span></div>

rank among the top ten leading causes of premature death, including heart
disease, stroke, and diabetes. While the obesity epidemic continues to impact
virtually all population groups, roughly 29% of children and 37% of adults in
South Central LA are obese, which is significantly higher than the rest of Los
Angeles County. LA County black and Latino resident’s fair worse when it comes
to obesity and obesity-related illnesses compared to whites and Asians. Thirty-two
percent of Latinos and thirty one percent of blacks are obese in LA County,
compared to 18% of whites and 8% of Asians. When it comes to South Central Los
Angeles black and Latino residents, who make up 10% and 87% of the population
respectively, the situation is no different. </span><br />

and becomes an oasis of fresh, healthy foods and inviting urban space. The
BUILD initiative attempts to build on past efforts to create an inviting
community where youth, families and adults can access healthy food, be
physically active outside, and are supported in maintaining a healthy lifestyle
that results in decreasing the prevalence of obesity and subsequently diabetes
and cardiovascular disease. Furthermore, this partnership believes in the
ambition and potential of the youth in this community. Central to this work, is
the commitment and emphasis on youth involvement, empowering youth to take
action towards improving social, environmental, and economic conditions in

purchasing cooperative and produce distribution program designed to meet the

<span style="font-family: Arial; font-size: 10.0pt; mso-bidi-font-family: Arial;">With little to no access to major
grocery stores in neighborhoods like Historic South Central Los Angeles, COMPRA
Foods aims to amplify the capacity of small convenience stores to carry
affordable and healthy produce. </span><span style="font-family: Arial; font-size: 10.0pt; mso-bidi-font-family: Arial; mso-fareast-font-family: "Times New Roman";">Currently, COMPRA is a grant funded initiative, supported by funders
such as USDA, the Department of Public Health, the Ahmanson Foundation, and the
Kresge Foundation. “We have a vision that COMPRA can become the next stand-alone
social enterprise, much like Amazon fresh, for local grocers and small convenience
stores.” This would mean more access to fresh, quality, fruits and vegetables at
a low-price for store owners, and residents alike, who frequent and shop at
these local stores. </span></div>

</span></b><span style="font-family: Arial; font-size: 10.0pt; mso-bidi-font-family: Arial; mso-fareast-font-family: "Times New Roman";">Once in the COMPRA Foods program,
small convenience store owners often see the value and cost savings of entering
the cooperative. This is due by enlarge to the fact that </span><span style="font-family: Arial; font-size: 10.0pt; mso-bidi-font-family: Arial;">small
food retailers source their food products from "big-box" grocers,
paying full price for these products and then marking them up for sale in their
own establishments. This often occurs because entrepreneurs lack relationships
with distributors who can deliver produce to their businesses at wholesale
prices. Small and mid-size farmers who may be interested in selling their
produce directly to these establishments also lack the relationships and
efficient distribution systems that can connect them to the hundreds of stores
in low-income, "food deserts”. This service gap between growers,
distribution companies, and small food retailers impacts low-income consumers
tremendously. Because there are few major grocery stores in some low-income
communities, families are required to buy food at elevated costs found at small
grocers who themselves lack low-cost alternatives to sourcing the produce they
offer. In contrast, by entering the COMPRA Foods program, small market owners
find support and are connected with small to large wholesale food distributors
that provide them with the accessibility and affordability to sell fresh,
healthy produce in their stores at a low-price to residents.<br style="mso-special-character: line-break;" />

</span></b><span style="font-family: Arial; font-size: 10.0pt; mso-bidi-font-family: Arial;">Through the BUILD Heath LA initiative, one part-time staff member will
be hired to manage and expand the COMPRA Foods program to include 16 small
markets within Historic South Central LA. Specifically, as a main focus in the
first year, youth Community Health Liaisons and the project partners will
conduct assessments of 22 local markets (out of the community’s 224 markets) to
determine their viability and interest in participating in a food purchasing
collaborative with expectations that a minimum of 16 will enter into the
collaborative by the end of the first year. After three months of training, the
anticipated goal is that each participating store will increase their healthy
food options as measured by a cooperative order tracking system.</span></div>

purveyors; and (4) deliver food weekly to more than 30 small grocers and

hospitalization can cause a momentary interruption of homelessness, but the
reality of receiving follow-up care and healing on the street is such that many
homeless patients find their way back into the hospital.</div>

<a href="http://nhfca.org/" target="_blank">National Health Foundation</a>’s (NHF) Pathway Recuperative Care
offers patients like Frank a safe and supported environment where they can
continue their healing journey and, thanks to our continuum Bridge Housing
program, find a permanent and stable home to return to upon discharge.</div>

Patients spend an average of 7-10 days in recuperative care
and this respite often triggers a desire in our guests to find a stable home;
however, the process is often challenging and takes a team of individuals to
connect the guest with the best possible permanent housing solution. Since our
program began in 2010, we have helped more than 3,000 formerly homeless
individuals heal and nearly half find their way home.</div>

On #GivingTuesday, we are inviting the community to donate
generously to NHF’s Pathways and Bridge Housing programs. Your donations will
provide taxi vouchers for patients to be transported from the hospital to our

currently attending Humboldt State University working towards her degree in
psychology and Robert is in his last year of high school and serves as a mentor

The harsh reality is only 40% of teen moms finish high
school and fewer than 2% finish college by age 30*. High school dropouts face
unemployment, poverty – and shorter lives. Yes, you read that correctly. There
is a significant link between education and health. High school graduates live
longer than high school dropouts. College graduates have even longer life
spans, better access to health care, better dietary and health practices, and
overall better health.</div>

On Friday, November 4<sup>th</sup>, 2016, we celebrated the
11<sup>th</sup> annual Hospital Heroes Awards luncheon. Together with our
Sponsors, <b style="mso-bidi-font-weight: normal;"><a href="http://california.providence.org/" target="_blank">Providence Health & Services, Southern California</a>, <a href="http://www.chla.org/" target="_blank">Children’s Hospital Los Angeles</a>, <a href="https://www.cedars-sinai.edu/" target="_blank">Cedars Sinai Hospital</a>, <a href="http://www.dvmc.com/" target="_blank">Desert Valley Hospital</a></b> and <b style="mso-bidi-font-weight: normal;"><a href="https://www.pvhmc.org/" target="_blank">PomonaValley Hospital Medical Center</a>, </b>we heard the stories of 31 heroes who, through
a deep commitment to care and community, have gone above and beyond the call of
duty as nurses, doctors, support staff, health care administrators and
volunteers. The task of choosing the three winners was challenging, to say the
least. </div>

What makes <b style="mso-bidi-font-weight: normal;">Shawn
Aguirre, </b>nurse educator at <b><a href="http://www.stjudemedicalcenter.org/" target="_blank">St. Jude Medical Center</a></b> a Hospital Hero? Perhaps Shawn's philosophy on
nursing says it best: “On or off duty, I am a nurse. My purpose is to heal.”
This past January, Shawn was on her way home when she saw two trucks, one
hoisted on top of the other and legs sticking out from under one of the trucks.
She immediately pulled over and ran across the street to see if she could help.
A fourth-year medical student was also on the scene and the two devised a plan;
he would go assess the young man’s legs and Shawn would climb under the truck
to determine the rest of his injuries. “I noticed his name tag was still on his
uniform, so I asked him if he knew his name,” Shawn said. “He was pale with
shallow breathing. He kept asking over and over how he got under the truck and
why his leg hurt so badly. He was clearly in shock.” For the next 30 minutes,
Shawn lay under the truck to calm him while waiting for the paramedics to
arrive. She did more than just assess his injuries clinically. She held his
hand. She told him it would be OK. Bryce Kulasxa joined Shawn at the Hospital
Heroes luncheon.</div>

RN, has worked the 7 pm – 7 am shift on the hematology-oncology floor of
<a href="http://www.chla.org/" target="_blank"><b>Children’s Hospital Los Angeles</b></a> since 1975.<span style="mso-spacerun: yes;">
</span>His tireless work ethic is driven by his motto: “I try to live in the
now and let the future develop around me. Many of my patients succumb to their
diseases. When there’s a remission or a recovery, we rejoice in that.”
Covington has remained in contact with many Children’s Hospital families. One
family, whose child died 20 years ago, joins him on fishing trips. “We email,
we talk, and we reminisce about their child we took care of and the love we
shared,” he says. “Love is the reason I do what I do. It’s painful
when you have a loss, but I love my job.” Upon receiving his award,
Tommy was visibly emotional, sharing that receiving this recognition, just
months before his retirement, was a beautiful way to celebrate his 46 years of
service. </div>

RN, BSN, MSN, from <b style="mso-bidi-font-weight: normal;"><a href="http://california.providence.org/san-pedro/" target="_blank">Providence Little Company of Mary Medical Center San Pedro</a>, </b>manages one of the most
challenging patient care units in the state, the sub-acute care center at
Providence Little Company of Mary Medical Center San Pedro. Patients are
transferred from hospitals throughout the state with gunshot wounds, terminal
cancer, and other life threatening conditions. She urges her nurses to never
give up on a patient and shares her motto, “Miracles do happen”. La Verna leads
her team with a sense of optimism resulting in amazing outcomes for patients
who were never expected to return home. “I’ve always liked working with these
patients who I believe have the greatest potential of getting better. I don’t
give up on them,” shares La Verna. La Verna manages this
125-bed specialty unit assuming both administrative and clinical oversight. La
Verna is on call seven days a week and regularly comes in on weekends and
evenings to meet with patient families and staff. She understands the value of
recognition for her team and special events for her patients and makes sure that
every birthday and holiday is a celebration. As we celebrated La Verna’s 27-year
career, she vowed to become more educated so she could make a greater
difference through her work.</div>

For <a href="http://nhfca.org/" target="_blank">National Health Foundation</a>, calling attention to the
work of the health care community is one way in which we express our gratitude
for the care and services of individuals, as well as for our partner hospitals.

Since 2005, <a href="http://nhfca.org/" target="_blank">National Health Foundation (NHF)</a> has reached out
to the Southern California health care community to nominate Hospital Heroes.
These are the individuals who, through their commitment to care, have left an
indelible mark on their patients, coworkers and communities. Since then, more
than 200 medical, technical, clinical, administrative and volunteer staff
members have been celebrated. The 11<sup>th</sup> Annual Hospital Heroes Awards
Luncheon will be held Friday, November 4<sup>th</sup>, 2016 and once again, we
will shine the light on the incredible service of the health care community. At
the event you will have the opportunity to meet heroes such as Debbie Keasler
who shares life-saving skills with her community over and above directing the
cardiac and stroke programs at Pomona Valley Hospital Medical Center.</div>

<div class="MsoNormal">
It takes a special person to direct one of the most
comprehensive, patient-centered and nationally recognized cardiac and stroke service
programs in the Los Angeles and San Bernardino Counties. Since 2001<b style="mso-bidi-font-weight: normal;">, Debbie Keasler</b>, RN, BS, MS, has done
all of this and more at <a href="https://www.pvhmc.org/" target="_blank"><b style="mso-bidi-font-weight: normal;">Pomona Valley Hospital Medical Center (PVHMC)</b></a>. A former cardiac critical care nurse, she
has seen both devastating and miraculous outcomes of patients with heart and
brain conditions. Debbie is much more than a director and clinical nurse; she
is frequently heralded as an “Unsung Hero” in the hospital. She is an avid
patient advocate, a skilled and experienced mentor to employees, and a kind
friend to many.</div>

<div class="MsoNormal">
Debbie works selflessly to put her patients and their
families first, including being available to answer questions and provide reassurance
24/7. No task is beneath her. She is a keen listener, advocate, and a quick
learner who always adapts to her patient’s changing and challenging needs
without question. Acts such as these have earned Debbie four awards from the
hospital’s Guardian Angel Recognition Program, which gives grateful patients
the opportunity to support the hospital while honoring a special health care
provider.</div>

Debbie is a champion for our community. She reaches out to
neighborhoods weekly to educate people on how to identify symptoms and
increased risks of heart disease and stroke and how they can live healthier
lifestyles. She teaches Hands-Only CPR to local schools, senior homes, and in the community to
provide others with tools that can save lives, and volunteers for the American
Heart Association. Debbie’s involvement is a testament to her steadfast
dedication to heart and vascular health. Debbie goes beyond the scope of her responsibilities to
support our communities and ensure that her patients receive compassionate
care. She’s a hero to all.</div>

Over the course of the ten years since we at <a href="http://nhfca.org/" target="_blank"><b style="mso-bidi-font-weight: normal;">National Health Foundation</b> (NHF)</a> have
been honoring the work of the Hospital Heroes among us, we have come to know many
staff members from one of our partner hospital networks in Southern California:
<a href="http://california.providence.org/" target="_blank">Providence Health & Services, Southern California.</a> This year we have 6 nominees from 6 of the Providence
hospitals. The Providence mission of compassionate care to the poor and
vulnerable is something that each of their nominees espouses, however, as
heroes, they have gone above and beyond their hospital’s mission to deliver
heartfelt care that has resonated with patients, their families and the
community.</div>

The 11<sup>th</sup> Annual Hospital Heroes Awards Luncheon
will be held this Friday, November 4<sup>th</sup>, 2016. Nominees will be
recognized and winners of the Hospital Heroes will be announced. It will be an
opportunity to meet these nominees:</div>

<div class="MsoNormal">
<b style="mso-bidi-font-weight: normal;">Carol Yokoyama</b>
has dedicated her career, and some would say her life to the mission of caring
for infants in the Neonatal Intensive Care Unit (NICU) at <a href="http://california.providence.org/torrance/" target="_blank"><b style="mso-bidi-font-weight: normal;">Providence Little Company of Mary Medical Center Torrance</b></a>. She has
been an employee here for 45 years. Difficult as it can be with this population
of delicate patients, the dedication and emotional bond a NICU nurse shares
with the family of a premature or sick infant is often the thread that holds
them together during the exhausting, heartbreaking journey a family experiences
when their child is born sick or too early. Pictures of past patients line the
walls in the break room and remind every one of this important work NICU nurses
do every day. Carol is one of these nurses. She is the kind of nurse who has
comforted mothers as they held their dying infants and the kind of nurse who
shares in the joy of an infant born four months early, finally going home.</div>

<b style="mso-bidi-font-weight: normal;">Alexis Schoffstall</b>
began working earlier this year as a kitchen worker at <a href="http://california.providence.org/saint-joseph/" target="_blank"><b style="mso-bidi-font-weight: normal;">Providence Saint Joseph Medical Center</b></a>. The mother of a little
toddler with autism, she was hired on a per diem basis for a new hospitality
program -<span style="mso-spacerun: yes;"> </span>delivering meals to patients,
providing coffee and other small comforts to visitors and offering a little
companionship. She proved a perfect fit, living the Providence core value of
compassion as she spent time with her patients, listening, chatting and
engaging. One of her patients was losing his battle with cancer at just 39, his
grieving mother rarely leaving his bedside, growing frail as she kept vigil. When
Alexis went home, she made sandwiches, cut up fruit and vegetables, bought
candy, and came back later that day, her child in tow to drop the food off for the
mother. She wanted to be anonymous. Co-workers told the woman a secret admirer
had dropped off the food. Hospital Chief Executive Julie Sprengel learned of
Alexis’s act of kindness and said she was so touched she was moved to tears.
“During this time of sadness and confusion across our nation and the world, we
can sometimes forget that every day there are people all around us
participating in selfless acts of kindness for each other and often for
complete strangers,” shared Sprengel in a note to the entire staff.</div>

<b style="mso-bidi-font-weight: normal;">Mary Smidderks</b>,
from <b style="mso-bidi-font-weight: normal;"><a href="http://california.providence.org/holy-cross/" target="_blank">Providence Holy Cross Medical Center</a>, </b>is a hero because she inspires patients and their families as well
as uplifts staff too! Fellow nurses have called her “a ray of sunshine to our
oncology patients” and her <i style="mso-bidi-font-style: normal;">Tea Lady</i> service,
“balm for the spirit”. <span style="mso-spacerun: yes;"> </span>Mary created her
own program putting her love of tea, linens, and fresh flowers to work. She met
with 12 different hospital leaders to share her plans. Together they created a ‘tea
service’ program for oncology and palliative care patients. “Teresa” was
recovering from extensive surgery for ovarian cancer and was understandably
withdrawn, depressed and in pain. Despite encouragement, she was reluctant to
get out of bed and take a few steps. Mary busied herself with preparing the tea
service while sharing her own story of how she learned to appreciate the
beautiful moments in life. Teresa selected a crocheted blanket for her lap, an
English Rose teacup, Chamomile tea and embroidered linen for her bedside table.
A small vase of flowers from Mary’s garden completed the transformation.</div>

As Teresa sipped tea, she smiled and became more animated,
sharing some of the details of her difficult journey. Later that day, two women
walking in the hallway; it was Teresa and her mother. They smiled and expressed
gratitude for the kindness offered by Mary. It was not medical facts that
helped Teresa, it was quite simply human interaction and the act of service
with compassion provided by the <i style="mso-bidi-font-style: normal;">Tea Lady</i>
that set Teresa on the road towards healing.</div>

known for the personal connection that she makes patients and their families
and has an uncanny way of remembering details about them. She is a fierce

a vegetative state. La Verna single handedly manages this 125-bed specialty
unit assuming both administrative and clinical oversight. La Verna is on call

<b style="mso-bidi-font-weight: normal;">Cara Armstrong</b>,
ED Clinical Nurse at <a href="http://california.providence.org/tarzana/" target="_blank"><b style="mso-bidi-font-weight: normal;">Providence Tarzana Medical Center</b></a> is a hero because she
inspires her whole team. She often works with patients who are homeless and in
need of more than just medical care. She takes her time with them, treating all
with dignity, and helping them to shower or shave and get fresh clothing. Recently,
she worked with an elderly couple in the ER on Christmas Day. The wife was
admitted in a delicate and weakened state and her husband was weary but
couldn’t bring himself to leave. Cara promised that his wife would not pass
away alone. After a period of long deliberation, the husband was eased and left
to rest with his family. As the wife’s condition continued to deteriorate, Cara
stayed by her side and she did not die alone. Cara was able to share beautiful
memories to help comfort the family. For Nurses Week, instead of receiving
gifts, Cara motivated her department to give a scholarship to Lily, a beloved
woman who helps clean rooms. She raised $3,000 toward a scholarship so Lily
could attend classes to become a certified nurse’s assistant. Cara convinced
doctors, clerks and others to contribute. Lily cried when she was surprised by
Cara and the team with a scholarship check. “I can’t believe it,” she said. “I
feel happy for my family too. I know it’s going to be different — for my son,
for my daughter, for my grandson, for my mom.” </div>

<b style="mso-bidi-font-weight: normal;">Dr. Brian Madden</b>
of the <b style="mso-bidi-font-weight: normal;"><a href="http://california.providence.org/saint-johns/" target="_blank">Providence St. John’s Health Center</a> </b>truly exemplifies the description of a Hospital Hero. He has made a
special and lasting contribution in his role as the Director of Palliative Care
Services in the past year. He provides compassionate care to patients who are
suffering and the family members who provide support under extremely difficult
circumstances. Dr. Madden has expanded the necessary scope of services at our
hospital to care for patients, especially in our Critical Care Units. He has
educated other physicians on the role of palliative care in providing needed
support for all patients, not just those with terminal illnesses. Brian is a
tireless champion who helps patients and families work with caregivers to make
informed decisions for themselves and their families. He has worked with
physicians both in and out of the hospital to educate them on the benefits of
palliative care for their patients and the making of difficult decisions. Dr.
Madden has expanded the scope of palliative care to include the entire
continuum of care, bringing his services into the outpatient arena, helping
patients and families understand their life choices and decisions at times that
may not be so emotionally stressful. Brian is a deeply caring, empathic, and

Since 2005, National Health Foundation has reached out to
the Southern California health care community to nominate Hospital Heroes. These
are individuals who, through their commitment to care, have left an indelible
mark on their patients, coworkers, and communities. Since then, more than 200
medical, technical, clinical, administrative and volunteer staff members have
been celebrated. </div>

The 11<sup>th</sup> Annual Hospital Heroes Awards Luncheon will
be held Friday, November 4<sup>th</sup>, 2016 and once again, we will shine the
light on the incredible service of the health care community and award winners
will be announced and recognized. </div>

This year’s nominees include David Watkins, associate
director of the Emergency Department (ED) of <a href="https://www.cedars-sinai.edu/">Cedars- Sinai</a> Hospital, Brian Lugo, Emergency
Preparedness Manager of <a href="http://www.dvmc.com/">Desert Valley Hospital</a>
and Tommy Covington, a hematology-oncology nurse at <a href="http://www.chla.org/">Children’s Hospital Los Angeles</a>.</div>

As ED nurse and leader at <a href="http://www.cedars-sinai.edu/">Cedars-Sinai</a>, David Watkins is an
inspiration to his peers, modeling outstanding patient- and family-centered
care that has him consistently going the extra mile for the people he serves. For
example, a recent patient needing medical attention refused to be admitted
without first finding someone to care for his dog. David made a promise to the
patient and personally walked and cared for the dog, in addition to ensuring
the dog had a safe place to stay until the patient was treated and safely
discharged several days later. </div>

It says “Emergency Preparedness Manager” on his badge but Brian
Lugo is much more than that, not only to the <a href="http://www.dvmc.com/">Desert
Valley Hospital</a> family, but to the whole community. For over 12 years Brian
has carved a place as an expert in all things emergency related. With
responsibilities at several hospitals in Southern California Brian must often
place himself in the center of chaos such as the recent Pilot and Blue Cut
Fires, and provide the staff with answers and solutions when his expertise is
needed. In his role as Chaplain, as he has done from the kindness of his
heart so many times, he has been that person who jumps in when a patient has
passed away. He comforts the family and offers that much needed shoulder to
lean on. </div>

Tommy Covington, RN, has worked the 7 pm – 7 am shift on the
hematology-oncology floor of <a href="http://www.chla.org/">Children’s Hospital
Los Angeles</a> since 1975. His tireless work ethic is driven by his motto: “I
try to live in the now and let the future develop around me.” Many of his patients
succumb to their diseases. “When there’s a remission or a recovery, we rejoice
in that.” Covington has remained in contact with many Children’s Hospital
families. One family, whose child died 20 years ago, joins him on fishing
trips. “We email, we talk, and we reminisce about their child we took care of
and the love we shared,” he says. “Love is the reason I do what I
do. It’s painful when you have a loss, but I love my job.”</div>

Frank, a 64-year-old Caucasian male, had a number of health
issues that needed immediate attention, but he was facing an even larger issue
than his failing health: he was homeless. For most of us who do have a place to
call home, it is highly likely that we have a doctor, dentist and perhaps an
urgent care clinic in our neighborhood that we feel we can turn to in the case
of need. For the homeless, these basic needs fall to the wayside until an
urgent health matter makes finding appropriate care a necessity and a challenge
best tackled by a group of caring individuals.</div>

When Frank came to <a href="http://nhfca.org/" target="_blank">National Health Foundation’s (NHF)</a> <a href="http://www.nhfca.org/pathawayrecupcare/" target="_blank">PathwayRecuperative Care</a> facility, he had been discharged from <a href="https://www.adventisthealth.org/white-memorial/pages/default.aspx" target="_blank">White MemorialHospital</a>, a local partner hospital, and was in need of follow-up care. Denise,
the LVN on Pathway’s staff immediately began the process of procuring a primary
care physician for Frank at the <a href="http://www.venicefamilyclinic.org/" target="_blank">Venice Family Clinic</a>, knowing that Frank would
need immediate support as well as prolonged follow-up for his chronic
conditions once he was discharged from recuperative care. Upon further
investigation, it was discovered that Frank would need to see a specialist for
surgery. This set in motion a flurry of activity that most individuals would
not think are part of accessing urgent medical care: A specialist from <a href="https://www.keckmedicine.org/" target="_blank">KeckMedicine of USC </a>agreed to perform surgery but insurance issues needed to be
addressed. In the mean time, Pathway staff worked with White Memorial to cover
an extended stay for Frank in recuperative care, knowing that his health would
deteriorate if he needed to wait for surgery whilst living on the street. With
an additional 4 months of support from White Memorial, the staff at Venice
Family Clinic then advocated for the patient and procured both the insurance
clearance needed as well as made sure that the surgery was scheduled in the
shortest time possible. With Frank’s most urgent medical needs met, there was
one thing left to do: find Frank a place to call home.</div>

Frank was the beneficiary of NHF’s Bridge Housing, a program
that allows patients to stay in recuperative care for recovery and for the time
that is needed to find a permanent housing solution. In Frank’s case, NHF was
able to procure permanent supportive housing for him and once his most pressing
health needs are under control, he will transition home.</div>

Frank’s case is not an isolated one. For every homeless
individual there is a health story that is unfolding. Thanks to partnerships
with area hospitals and clinics, NHF is able to provide dignified and holistic

In 2015, <a href="http://nhfca.org/" target="_blank">National Health Foundation</a> (NHF), <a href="http://www.dignityhealth.org/californiahospital/" target="_blank">CaliforniaHospital Medical Center </a>(CHMC), and the <a href="http://publichealth.lacounty.gov/" target="_blank">Los Angeles County Department of PublicHealth</a> (LACDPH) were awarded a <a href="http://buildhealthchallenge.org/" target="_blank">BUILD Health Challenge</a> planning grant to
launch the BUILD Health LA initiative. The novel partnership had a specific
goal: address the complex issues of obesity, diabetes and cardiovascular
disease. Turning our attention to Historic South Central Los Angeles, the
partnership sought to identify upstream solutions that address health
disparities and inequities in the neighborhood by addressing the root causes of
these issues. A team of nine emerging youth leaders from three South LA high
schools was selected as community liaisons and led the comprehensive data
collection efforts that informed the solutions identified by a community action
plan.

Following 23 key informant interviews, 51 references and 515
community surveys, the Youth-Driven Healthy South LA (BUILD Health LA) team
identified a lack of access to healthy food, a lack of access to open space for
physical activity, and safety concerns as the barriers to a healthy lifestyle
for their community. The community liaisons, together with community partners,
are now moving to implement innovative and upstream solutions that tackle these
issues. </div>

In a densely populated community unhealthy ‘junk’ food is
available at an alarmingly disproportionate rate, prompting some to label South
LA as a “food swamp.” A strategy to provide 16 corner store owners the
opportunity to enroll in the <a href="http://lurnetwork.org/" target="_blank">Leadership for Urban Renewal Network’s </a>(LURN) <a href="http://goodfoodla.org/wp-content/uploads/2015/10/New_COMPRA_2p.pdf" target="_blank">CommunityMarkets Purchasing Real and Affordable </a>(COMPRA) Foods Program will infuse more
healthy food options into the neighborhood. The purchasing program will allow
market owners the opportunity to purchase fresh produce at a low-cost and
thereby increase access to fresh fruit and produce for the community. The youth
community health liaisons will identify the 16 markets and, with the support of
the LACDPH, will help the markets to promote healthy food messaging at the
stores.</div>

With only .4 acres of park space per 100,000 residents,
South Los Angeles stands in stark contrast with the recommended 3 acres per
1,000 people. Furthermore, residents report that the 10 parks and 4 pocket
parks that do exist are often not maintained and are centers of crime making
them unsafe for residents. The youth community health liaisons will take the
lead in developing and advocating for park safety. With a keen eye on
pedestrian and bicycle accessibility, the youth will conduct assessments to
determine what types of infrastructure are needed to ensure all parks are safe
and inviting to the community. The youth will then present their recommendations
to local stakeholders to advocate for action.</div>

The environmental scan further discovered that recreational
facilities offer a limited number of affordable physical activity programs, and
that the programs that are offered often have long waiting lists. Providing
residents opportunities to engage in physical activity is crucial especially
given the high numbers of obesity, diabetes and other disease related
to sedentary lifestyles and inactivity. The project team will expand the
capacity of a few local community-based organizations to offer additional
physical activity programs for community members, specifically in park-poor
areas of the neighborhood and areas with limited fitness programming, by
connecting these organizations to <a href="http://www.csun.edu/health-human-development/kinesiology/news/3-wins-fitness-healthier-california-communities" target="_blank">California State University Northridge’s3WINS Fitness Program</a>. These trained kinesiology students will teach free
fitness classes that will be held outside in local parks near the host
organizations. <a href="http://www.apch.org/" target="_blank">A Place Called Home </a>and <a href="https://allpeoplescc.org/" target="_blank">All People’s Community Center</a> will
outreach to residents to participate in these new programs.

With the plan in place, the youth and the community partners
share a common goal: getting the word out about the healthy changes in the
neighborhood. Currently, the community is saturated with unhealthy food
messages on billboards, buses and street corners but the youth and the project
partners have identified more than 70 service organizations that have regular
contact with residents and could help maximize the awareness of and participation
in this program. The youth will develop a comprehensive marketing campaign and,
together with the project partners, will raise awareness of the health-related
improvements in their community. The sustainability of policy, systems and the
health of their neighbors are depending on their ability to connect residents
to the fresh produce, safe parks and opportunities for fun physical activity
sprouting up in their community.</div>

<a class="linkOnClick" data-include="300327505" href="http://www.usda.gov/wps/portal/usda/usdahome" rel="nofollow" target="_blank">U.S. Department of Agriculture</a> (USDA) Administrator <span class="xn-person" itemscope="" itemtype="http://schema.org/Person"><span itemprop="name">Audrey Rowe</span></span> met with youth and leadership of the <a class="linkOnClick" data-include="300327505" href="http://nhfca.org/" rel="nofollow" target="_blank">National Health Foundation's</a>
(NHF) innovative youth-led Health Academy program to highlight
successful implementation of the Supplemental Nutrition Assistance
Program –Education (SNAP-Ed) funded Champions for Change grant,
administered locally by the Los Angeles County Public Health
Department. The goal of SNAP-Ed is to improve the likelihood that
persons eligible for SNAP will make healthy food choices within a
limited budget and choose physically active lifestyles. </div>

The <a class="linkOnClick" data-include="300327505" href="http://www.nhfca.org/healthacademy/" rel="nofollow" target="_blank">Health Academy</a>,
a youth-driven nutrition education and obesity prevention program, has
been an opportunity for youth at Thomas Jefferson High School in <span class="xn-location" itemprop="contentLocation" itemscope="" itemtype="http://schema.org/Place"><span itemprop="geo" itemscope="" itemtype="http://schema.org/address"><span itemprop="addressLocality">South Los Angeles</span></span></span>
to create and implement upstream interventions to improve the health of
their community. The focus of Rowe's visit will be the following Health
Academy successes: The Mini Farm-Stand/Breakfast in the Classroom food
waste abatement project, the school cafeteria makeover and the Healthy
Marketing and Product placement corner store makeover projects. "We are
honored to have been a part of Administrator Rowe's visit to southern <span class="xn-location" itemprop="contentLocation" itemscope="" itemtype="http://schema.org/Place"><span itemprop="geo" itemscope="" itemtype="http://schema.org/address"><span itemprop="addressLocality">California</span></span></span>.
The youth involved in Health Academy have impacted the health of their
community in tangible ways that are rippling out beyond the school's
walls. The enthusiasm of the Health Academy students is touching
families and friends and creating a veritable shift in consciousness

<b>About the Health Academy Projects </b>The
Mini Farm-Stand project was designed by students to provide classmates
access to healthy snacks while curbing food waste. Students placed
attractive baskets in pilot classrooms and filled them with the fruit
and foods left over from the Breakfast in the Classroom program.
Typically, these foods would have been discarded, however, by placing
them in the Farm-Stand baskets, students were able to simply take a
piece of fruit at any time in the day, as needed. The results of the
pilot were remarkable. Food waste was significantly reduced and students
greatly appreciated having access to healthy options to the typical
snack bar and vending machine fare offered between school meals. The
Mini Farm-Stand project has been expanded through two Los Angeles
Unified School District (LAUSD) schools and is recognized as a potential
model for district-wide implementation.</div>

Students in the Health
Academy also tackled the issue of low participation in the school lunch
program. The youth polled their fellow students and learned that the
layout and appearance of the cafeteria was not conducive to purchasing
and consuming a meal in the allotted 30-minute lunch break and that many
students were consuming snack foods for lunch rather than tackle the
lunch lines. With a few minor tweaks to the layout and a creative
point-of-purchase marketing scheme complete with posters touting the
value of school lunches, the youth were able to significantly increase
participation in the school lunch program.</div>

The Health Academy youth
shifted their attention to the community around their school and have
begun to offer local merchants 'Store Makeovers'. Recognizing that <span class="xn-location" itemprop="contentLocation" itemscope="" itemtype="http://schema.org/Place"><span itemprop="geo" itemscope="" itemtype="http://schema.org/address"><span itemprop="addressLocality">South Los Angeles</span></span></span>,
with its high number of fast-food restaurants and liquor stores
constitutes a food desert, the students set out to make fresh produce
and water available at the corner stores closest to their school.
Students worked with <span class="xn-person" itemscope="" itemtype="http://schema.org/Person"><span itemprop="name">Mercado Garibaldi</span></span>
owner, Joel, to increase the visibility of bottled water as an option
to sugary beverages, and they worked with La Favorita owner, Carlos, to
increase the availability of fresh fruits and vegetables. Both retailers
embraced the changes and the energy of the students and plan to
continue offering healthy options
year-around. </div>

school-based teen pregnancy prevention program for adolescent females at-risk
for a first time pregnancy. The program focuses on healthy behaviors, family
planning, skill development, communication, and goal oriented behavior to
empower teen girls to prevent unplanned pregnancies and improve their overall
health, well-being, and success in life.</span></i></div>

<span style="font-size: 12.0pt;">We recently caught up with
one of our <i>Be a STAR </i>graduates, Ty’Kese, before she headed off to
college. Ty’kese epitomizes what Be a STAR is all about. She is a <b><i>S</i></b><i>uccessful
<b>T</b>een <b>A</b>cting <b>R</b>esponsibly</i>. Pregnancy and parenthood
cause many young, promising teens to drop out of school. And dropping out of
school can completely devastate their lives and shut down otherwise bright
futures. </span></div>

<span style="font-size: 12.0pt;">High school drop-outs face
unemployment, poverty – and shorter lives. Yes, you read that correctly. There
is a significant link between education and health. High school graduates live
longer than high school dropouts. College graduates have even longer life spans,
better access to health care, better dietary and health practices, and overall
better health.</span></div>

<span style="font-size: 12.0pt;">Ty’Kese is smart and
interested in making a difference in others’ lives. She’s a vegetarian. She
wants to study psychology and social work as an undergraduate and then earn her
master’s degree. She prides herself on asking questions and being well informed
in the classroom and in the world at large.</span></div>

<span style="font-size: 12.0pt;">Ty’Kese was surprised when
the social worker at her high school recommended she get involved with NHF’s <i style="mso-bidi-font-style: normal;">Be a STAR girls</i> program. Although she’d
had a few pregnancy scares, she really didn’t think she was at-risk of becoming
pregnant. Plus, helping to take care of her younger brother at home, she felt,
gave her a good idea of what it would be like to be a young mother. It didn’t
seem like it would be that hard.</span></div>

<span style="font-size: 12.0pt;">The truth is she didn’t want
anything to do with <i style="mso-bidi-font-style: normal;">Be a STAR girls</i>,
but she sat in on one session and was hooked. She loved getting honest, factual
information about health and sexuality. She loved being in an atmosphere where
she could freely ask questions. She appreciated having the opportunity to learn
more about her own body in a safe environment with other girls. And it was eye
opening for her to have myths such as, "you can’t become pregnant when you have
your period" debunked.</span></div>

<span style="font-size: 12.0pt;">What is shocking is that Ty’Kese
had taken several other health and sex education classes and said she learned
more in just a couple of Be a STAR sessions than she ever learned before. Be a
STAR prepared her for adulthood by giving her full information about her birth
control options and helping her set forth her personal goals for the next two,
five, and ten years. </span></div>

<span style="font-size: 12.0pt;">The goal setting exercises
caught her attention. Yes, she enjoyed helping take care of her little brother,
but now she saw how completely different this would be from actually being a
young mom. In one exercise, Ty’Kese created a daily schedule of what her life
would be like as a parent. This “as if” schedule showed her an unending string
of early mornings, grueling days, and the difficulty of getting someone to
watch her baby while she went to school. She could see how extremely difficult,
if not impossible, it would be to reach her goals in these circumstances.</span></div>

<span style="font-size: 12.0pt;">Ty’Kese is more concerned, careful,
and knowledgeable about birth control than she used to be. Her eyes are wide
open. She pays attention to her own body and is self-aware. She wishes Be a
STAR was required for all girls at her former high school. </span></div>

<span style="font-size: 12.0pt;">She wants to be a mom one
day, when she’s ready, perhaps in a decade, after she has completed her
education and has a good job as a social worker. For now, Ty’Kese wants to
enjoy being a student and focus on her goals for a life that will be filled
with achievements and the prospect of long-term health.</span></div>
